Beverley PCN has a number of clinical professionals that help deliver services to the patient population of the 4 member practices.

That delivery could be within Practices, at a care home or residential home site or one of the many housebound patients of the 4 member practices.

The team currently cover Beverley and Cottingham with 11 Care Homes and 5 Residential establishments within the area. As part of our current team we have:

About the roles

Book appointments with the most appropriate person to treat your ailment or condition.

Carry out clinical exams, make diagnoses based on assessment of patient, take specimens, create treatment plans and prescribe medication based on test findings, provide post surgical care to patients, perform routine medical procedures, complete evaluations of patients and referring to specialists as required.

Take blood, do weight management, take and monitor blood pressures, perform new patient and NHS health checks, ear syringing, basic wound care, administer vaccines, B12 injections, spirometry, long term condition reviews, INR monitoring, health promotion, home visiting, chaperoning duties.

Assessment and triage of patients, same day triage, define treatment plan, advise patients on general healthcare, promote self–management, perform specialist health checks and reviews, perform and interpret ECGs, undertake investigatory procedures, blood tests, refer to GP if required, organise medication, home and care home visits.

Complete complex patient reviews/assessments, diagnose acute and chronic conditions, treatment of minor illnesses, prescribe and review medication, advise on prescribed medication, evaluate treatments plans, manage conditions holistically, ensure joined up care for patients.

Treat and diagnose all common medical conditions, refer patients to hospital or other medical services for urgent or specialist treatment.

Help patients with musculoskeletal issues such as back, neck and joint pain by assessing and diagnosing issues, give expert advice on how to best manage their conditions, refer them on to specialist services if required, provide direct access without waiting for a referral.

Carry out structured medication reviews, health checks, management of long term conditions, provide advice for those on multiple medicines, provide a prescription management service.

Help patients quickly and directly identify the help they need for non medical issues, contact patients and do initial consultations, assess social needs, refer to other services as appropriate, update patient notes, refer to GP as appropriate.

Meet the eye care needs of patients, provide consultation, specialist imaging and diagnostics, elective and emergency medical and surgical treatments, follow up care, monitoring of eye conditions, delivery of long term treatment programmes.

Experts in providing mental health care, access to talking therapies, mental Health support for those waiting for IAPT services, physical health support for those with mental health problems, low level mental health support for those with long term conditions, ongoing support for those that have been discharged from secondary care, management of medications.

Senior doctors that have completed medical training in a specialised area of medicine, have patients referred to them for specific medical issues that are in their speciality.
