Beverley PCN’s goal is to ensure everyone has an understanding of what patient services we are offering for our patient population.
We believe that a collaborative approach by all of the practices within the PCN will allow us to make best use of a varied skill mix we have available to us and will allow us to ensure our patient population can have access to the most appropriate person, to deliver the most appropriate care to them, at all times.
Beverley PCN’s main population health challenges 2023 – 2024
- CHD/Hypertension / STIA – promotion of healthy lifestyle initiatives along with early detection of at-risk people
- Cancer – early diagnosis and long-term remission
- Dementia – Offering suitable care and support services
- MSK – a specialist physiotherapist that can deliver end to end care from diagnosis to treatment.
- Mental Health – trained professionals to help with mental health issues ranging from social needs, health and well-being needs to support with poor mental health
- Diabetes – focus on prevention of diabetes and putting diabetics into remission.

How we can help
On initial call to each practice, a patient will speak to a Care Navigator.
The Care Navigator will take details of the reason for your call to ensure that you are seen by the person most able to help you.
For example: – Pain in your knee – a direct appointment to a FCP MSK would be your most appropriate clinician as they can diagnose, refer to a consultant and offer pain relieving injections if required.
For example: – possible mental health concerns, caused by a single or several issues (financial, medical, home life, work life, lifestyle). It may be, that dependant on your concerns your best route would be to speak to a social prescriber, health and wellbeing coach or the mental health and wellbeing coach
